Eligibility - OEBB Members
We have partnered with the Oregon Educators Benefit Board (OEBB) to offer the Prevent Diabetes program to their adult members who are at risk for developing diabetes.
In addition to confirming your OEBB membership, our application asks for information to determine your clinical eligibility for the program.
- Your height and weight are used to calculate your Body Mass Index (BMI) number.
- An included CDC prediabetes risk test questionnaire is used to determine your CDC risk score.
Please read below for more information on determining eligibility for the Prevent Diabetes program.
OEBB Moda members
For OEBB members who use Moda as their medical insurance, please provide your Moda ID number.
The Moda ID number can be found on the front of your id card as shown in the image below:
OEBB Kaiser Permanente members
For OEBB members who use Kaiser Permanente as their medical insurance, please provide the Medical Record Number found on the front of your Kaiser Permanente ID card. This number is all numeric and is 5-8 digits long.
Body Mass Index (BMI)
Your BMI number must be at 30 or above OR between 25-30 with a listed long term condition (such as hypertension) in order to participate in Prevent Diabetes.

CDC Prediabetes Risk score
Your CDC score must be a 5 or higher according to the CDC Prediabetes risk test in order to participate in Prevent Diabetes.
