Eligibility - PEBB Members
We have partnered with the Public Employees' Benefit Board (PEBB) to offer Prevent Diabetes to their adult members who are at risk for developing diabetes.
In addition to confirming your PEBB membership, our application asks for information to determine your clinical eligibility for the program.
- Your height and weight are used to calculate your Body Mass Index (BMI) number.
- An included CDC prediabetes risk test questionnaire is used to determine your CDC risk score.
Please read below for more information on determining eligibility for the Prevent Diabetes program.
PEBB Benefit Number
This is an 8 digit number that starts with a "P" provided to you by PEBB on your PEBB member statement. We use this number to confirm your status as an active PEBB member.
Please note, this is not the same number that is on the front of your membership card.

Body Mass Index (BMI)
Your BMI number must be at 30 or above OR between 25-30 with a listed long term condition (such as hypertension) in order to participate in Prevent Diabetes.
You can calculate your BMI number on this website.
CDC Prediabetes Risk score
Your CDC score must be a 5 or higher according to the CDC Prediabetes risk test in order to participate in Prevent Diabetes.
